Contact the Clerk of Courts for the county where the arrest occured. Copies are $1.00 per page, and certification is $1.00 per document.
You have to contact the Clerk of the court where your DUI was heard. It was probably the county Superior Court. You can get the phone number for the Clerk. Call and ask what the fee will be and how you submit a request for a certified copy now that you live out-of-state. You’ll probably have to make a written request with the fee included.
You can also get a copy online from a database that maintains and updates a criminal history database.
